Key Responsibilities

As a Care Coordination Support Specialist, you’ll play a pivotal role in ensuring seamless coordination between patients, providers, and external agencies. Your tasks will include:

  • Onboarding Operations: Retrieve insurance and address details from public databases, monitor visit logs for missed appointments, verify EHR configurations, and update patient contact information.
  • Post-Visit Updates: Attach clinical and nonclinical addendums to care plans, send care plan links via EHR, review visit notes, and track labs, diagnostic imaging, referrals, and durable medical equipment (DME) orders.
  • External Communications: Prepare patient notification letters using mail merge, maintain Excel trackers for external agencies, and send weekly stakeholder updates.
  • Care Coordination Support: Review EHR for new referrals, labs, and DME; monitor incoming records; and follow up on overdue items with onshore teams or vendors.
  • Additional Duties: Execute daily tasks assigned by supervisors and contribute to process improvement initiatives in care coordination workflows.

Qualifications

To excel in this role, candidates should meet the following requirements:

  • Proven experience in healthcare quality operations.
  • Strong communication skills and a collaborative mindset.
  • Proficiency in Microsoft Excel for data tracking and reporting.
  • Prior experience with Electronic Health Records (EHR) systems.
  • Ability to understand and relay medical information and facility protocols.
  • Detail-oriented with a passion for improving healthcare delivery.
